Knee Surgery Overview

Knee replacement is a routine surgery performed on over 600,000 people worldwide each year

In America the average joint replacement patient is around 65-70 years old

however patients of all ages receive knee implants.

Many people are born with a natural tendency toward knock knees or bowleggedness, severe arthritis can lead

to an uncomfortable degree of misalignment, one of the goals of total knee replacement is to

restore your body's natural alignment. 

Knee replacement risks include blood clots in the legs that can travel to the lungs (pulmonary embolism).

Pulmonary embolism can cause shortness of breath, chest pain, and even shock.

Other risks include urinary tract infection, nausea and vomiting (usually related to pain medication)

chronic knee pain and stiffness, bleeding into the knee joint, nerve damage, blood vessel injury

and infection of the knee which can require re-operation.

Furthermore, the risks of anesthesia include potential heart, lung, kidney, and liver damage.

Before surgery, joints adjacent to the diseased knee are carefully evaluated to ensure optimal outcome from the surgery.

All medications which the patient is taking are reviewed.

Blood thinning medications such as warfarin (Coumadin), and anti-inflammatory medications such as Aspirin

may have to be adjusted or discontinued prior to surgery.

Routine blood tests of liver and kidney function, and urine tests are evaluated for signs of

anemia infection or abnormal metabolism.

Chest x-ray and EKG are performed to exclude significant heart and lung disease which may preclude surgery or

anesthesia, if a patient's weight is greater than 200 pounds he/she is less likely to have good long-term results

excess body weight simply puts the replaced knee at an increased risk of loosening and/or dislocation.

Pre-Op Requisites

To minimize the risk factors THAIMEDCO requires that you undertake a full set of medical examinations in the USA

as described above and supply same to THAIMEDCO for its evaluation as to your suitability.

Documentation from your regular doctor or medical practitioner stating that you are fit and that you are physically

able to handle surgery of this type is a further pre-requisite.

These same medical examinations will be repeated upon your arrival in Thailand.

Because blood transfusions could be needed during your surgery you may wish to donate

several units of your own blood.

What happens on the big day?

On the day of surgery, a small tube (intravenous line) will be inserted into your arm, this tube will be used to

administer antibiotics and other medication during your surgery.

You will then be taken to the operating room and given anesthesia, after the anesthesia takes effect

your knee will be sterilized, the surgery will begin with an incision over the knee that will expose the joint.

When the bones are fully visible to the surgeon, special, precision guides and instruments are used to remove

the damaged surfaces and shape the ends of the bones to accept the implants.

The implants are then secured to the bones, it may also be necessary to adjust the ligaments that surround

the knee in order to achieve the best possible knee function.

When the surgeon is satisfied with the fit and function of the implants, the incision will be closed, a special drain may be

inserted into the wound to drain the fluids that naturally develop at the surgical site.

A sterile bandage will then be applied, and you will be taken to the recovery room where you will be closely monitored.

Knee replacement generally requires between one and one-half to three hours of operative time.

After surgery, patients are taken to a recovery room, where vital organs are frequently monitored.

When stabilized, patients are returned to their hospital room.

Passage of urine can be difficult in the immediate postoperative period, and this condition can be aggravated by pain

medications, acatheter inserted into the urethra (a Foley catheter) allows free passage of urine until the patient

becomes more mobile.

Physical therapy is an extremely important part of rehabilitation and requires full participation by the patient for optimal

outcome, patients are encouraged to begin physical therapy forty-eight hours after surgery and to begin walking

a few steps with the help of a walker.

Some degree of pain, discomfort, and stiffness can be expected during the early days of physical therapy.

Knee immobilizers are used in order to stabilize the knee while undergoing physical therapy, walking, and sleeping.

They may be removed under the guidance of the therapist for various portions of physical therapy.

You can expect to stay in the hospital for about 5-7 days after surgery,

When your surgeon determines that you have recovered sufficiently you will be discharged or you may be

transferred to a specialized rehabilitation facility if you are experiencing movement/walking difficulties.

Post Operative Therapy

At THAIMEDCO's Five Star hotel rehabilitation accomodations your personal THAIMEDCO rehabilitation therapist

will introduce you to a program of appropriate exercises designed to allow you to regain balance, walk with crutches

regain knee related muscle suppleness & improve movement in your knee to assist in your walking posture.

your personal THAIMEDCO therapist will continue to work with you for the first 2 weeks of our four week rehabilitation

program, advanced rehabilitation care will include swimming & limited walking activities,

step negotiation and proper bending techniques.

Driving a car can generally commence four weeks after surgery.

The Don'ts

Successful knee replacement surgery relieves pain and stiffness allowing you to resume many

of your normal daily activities, after you have fully recovered from your surgery you will still have some restrictions.

Normal daily activities do not include contact sports or activities that put excessive strain on your joints.

Avoid repetitive heavy lifting Avoid excessive stair climbing

Maintain appropriate weight Stay healthy & active

Avoid "impact loading" sports such as jogging downhill skiing & high impact aerobics

Consult your surgeon before beginning any new sport or activity

Think before you move

Avoid any physical activities involving quick stop-start motion, twisting or impact stresses

Avoid excessive bending when weight bearing, like climbing steep stairs

Dont lift or push heavy objects Dont kneel Avoid low seating surfaces & chairs

Hip Replacement

Age, injury or disease can cause hip joints to "wear out”" when their hips "wear out" many people have pain

that limits their ability to walk, climb stairs, and do activities they enjoy. If this happens to you, a total hip replacement

surgery is one option you should consider.

This surgery can relieve the pain so that you can again take part in the activities you enjoy.

The Normal Hip

The hip joint is a ball and socket joint, the ball is formed by the head of the thigh bone (femur) the socket is part of the

pelvis called the acetabulum, in a normal hip, the ball and socket are covered with a smooth layer of tissue called cartilage.

The cartilage allows the ball to glide easily inside the socket & cushions your hip joint, muscles & ligaments hold your

hip joint in place, when surrounding muscles support your weight the joint moves smoothly allowing you to walk without pain.

The Problem Hip

In the problem hip, worn cartilage no longer serves as a cushion, age, injury or disease can wear away the cartilage.

As the cartilage wears away from the bones, the bones rub together creating rough surfaces like sandpaper

on the bones, when you move your leg, the rough ball grinds in the socket causing pain and stiffness.

As the pain worsens you avoid using the joint, causing the muscles to weaken, as your muscles weaken

your hip joint is less able to support your body weight and it feels unstable.

The New Hip

Total hip replacement surgery removes damaged bone and cartilage from the hip joint and replaces both the bone

and cartilage with an artificial joint (prosthesis).

An artificial ball connected to a stem replaces the ball of your thighbone.

An artificial cup, shaped like a bowl, replaces the worn socket.

These parts connect to create a new artificial hip that works almost exactly like your own hip joint.

All parts have smooth surfaces for comfortable movement once you have healed from surgery.

The prosthesis is made of wear-resistant plastic called pethylene and metal (titanium or stainless steel).
